Census track ensures each resident is tracked from the day of admission to make sure there is always a payer source in place.
Each facility will be assigned a designated census track employee as their primary contact person.
The Tracker will conduct a weekly call to determine all Medicare, Private and Medicaid Pending Patients and determine their status.
Upon completion of the call a task list will be created and sent to all relevant parties.
The tracker will diligently pursue that the respective personnel take responsibility and complete the tasks assigned to them.
